The Financial Industry Regulatory Authority (FINRA) suspended Karen Lee Chafe of Melbourne Beach, Florida, for a year, effective June 16, 2014, and assessed a fine of $5,000 based on findings that she engaged in a practice of modifying and submitting previously used forms for withdrawals from annuities, and individual retirement accounts (IRAs).
Instead of having customers execute new withdrawal requests for withdrawals, FINRA found that she reused old forms, whiting out the old information and substituting new dates, amounts, etc. and submitted the forms to her firm and the annuity companies as originals.
According to FINRA records, Chafe was registered with Berthel, Fisher & Company Financial Services from 12/2002-01/2012.
